Filling out TLC form request

1. Go to Forms tab, click My Forms

Under My Forms, click + New Telework Request button to open a new form

2. Select your Supervisor/Manager from the Your Supervisor's Name field

3. Use the date picker to select a Requested Start Date that you’d like to start teleworking.

4. Enter in the start time and end time in the Workday Begins and Workday Ends fields (Do this for Week 1 and Week 2)

5. Select the Telework Day(s) that you plan to work (can choose one or multiple days)

6. Under Remote Computer Access section, select one choice that applies to your access needs

  • I need access to my work computer and/or the COA network (G drive, etc.) from home.

  • I am already able to access my work computer and/or the COA network from home through GoToMyPC, NetMotion, or Citrix.

  • I do not need to access my work computer or the COA network from home. I understand I can access my Outlook email account online and and my voicemail remotely.

7. Under the Employee Acknowledgement section, you are required to review and understand: "City of Austin Flexible Work Arrangement Procedures" and "TPW Telework Procedures".

8. Digitally sign the Employee Signature field

  • Checking the box, I verify that I am the employee listed below

  • Type out Name for Employee eSignature

  1. When complete, click Submit at the bottom of the form

10. An email notification will be sent to confirm that your request has been submitted.
