Who should use this form
ATPW Division Managers that have talked to a staff member and supervisor and identified them as a candidate that has the desire and availability to participate in the Awards and Recognition Program Committee.
These are the eligibility requirements:
Not currently on a Performance Improvement Plan (PIP).
No active COA policy or ethics violations under review by Human Resources.
No disciplinary action within the past 6 months.
In "good standing" as defined by the Municipal Civil Service Rules of the City of Austin.
Have at least six (6) months of continuous service with the department.
Have a recently submitted SSPR, with an overall rating of ‘Successful' or higher, and no individual priority below 'Successful.'
Demonstrate a positive and inclusive attitude that fosters a culture of appreciation and respect.
Show fairness, objectivity, and a non-biased, inclusive approach to both peers and the community.
Demonstrate or have the ability to maintain strict confidentiality in the workplace.

Filling out TPW Awards & Recognitions Committee Member Nomination Form
Section 1 - Nominee Information
For Employee Name - select your name from drop down or start typing first and last name to find your name [required]
For Supervisor Name - start typing first and last name to find your Supervisor's name [required]
Section 2 - Reason for Nomination
In the Reason field, please provide a brief explanation (3–5 sentences) describing why you believe this employee is a strong candidate to represent your division on the TPW Awards & Recognition Committee. Please highlight any relevant experience or qualities related to: leadership, collaboration, initiative, accountability, communication, planning, budgeting, reliability, administrative support, or familiarity with technological tools and systems (Microsoft Office or SharePoint). Consider how this employee could contribute to the success of the committee in a way that aligns with their strengths.
You must provide enough information in this field. There is a 150 character minimum that must be met, you will see an error message preventing you from submitting. See error message below.
