Committee Acknowledgment

If selected for Awards and Recognition Committee, an email notification will sent to the self-nomination requester.

Email Notification of Nomination Decision

Accessing the Committee Acknowledgement Form

  • Click the Commitment Membership Agreement link

  • It will open up a web browser to the A&R Committee Acknowledgement page

  • Click Save and Continue

  • Click the link beside the File field to launch the policy document

Reviewing Policy Document

  • Read Acknowledgment file(s) attached/linked

Signing the Employee Acknowledgment

  • Fill out

    • Please enter your Supervisor's name - start typing first and last name to find your Supervisor's name and select them [required]​

    • Employee Verification [required]​

    • Employee eSignature (Please type your Name as it appears on your Human Resources paperwork) [required]​

  • Click Submit when finished

Confirmation

  • Upon completion, you'll see a confirmation page
