Committee Self-Nomination

Who should use this form

ATPW staff that have the desire and availability to participate in the Awards and Recognition Program Committee. These are the eligibility requirements:

  • Not currently on a Performance Improvement Plan (PIP).

  • No active COA policy or ethics violations under review by Human Resources.

  • No disciplinary action within the past 6 months.

  • In "good standing" as defined by the Municipal Civil Service Rules of the City of Austin.

  • Have at least six (6) months of continuous service with the department.

  • Have a recently submitted SSPR, with an overall rating of ‘Successful' or higher, and no individual priority below 'Successful.'

  • Demonstrate a positive and inclusive attitude that fosters a culture of appreciation and respect.

  • Show fairness, objectivity, and a non-biased, inclusive approach to both peers and the community.

  • Demonstrate or have the ability to maintain strict confidentiality in the workplace.

  • Go to Programs tab, click Awards and Recognition

  • Click on large Committee Self-Nomination button

Filling out TPW Awards & Recognitions Committee Member Self-Nomination Form

Section 1 - Nominee Information

  • For Employee Name - select your name from drop down or start typing first and last name to find your name [required]​

  • For Supervisor Name - start typing first and last name to find your Supervisor's name [required]​

Section 2 - Reason for Nomination

  • In the Reason field, please provide a brief explanation (3–5 sentences) describing why you believe you are a strong candidate to represent your division on the TPW Awards & Recognition Committee. Note: Consider your background and experience in collaboration and supporting a culture of appreciation

  • Click Save and Continue to proceed to the next page

Self-Nomination Attestation Page

  • Please review this page, read the "Nomination Attestation" section and acknowledge below

  • Fill out

    • Employee Verification [required]​

    • Employee eSignature (Please type your Name as it appears on your Human Resources paperwork) [required]​

  • Click Submit when finished

Your nomination form will go through a formal review process with a Human Resource Review, Supervisor Review, Division Managers Review and Executive Review.
